Requesting Medical Records
St. Rose Dominican Hospitals' Health Information Management Department maintains all patient medical records. The medical record is a permanent document of the history and progress of every patient's medical care and is a compilation of observations and findings recorded by the patient's physician and members of the hospital healthcare staff.
Records are used in a variety of ways:
- to provide continuity of patient care,
- to verify insurance claims for medical care rendered,
- as a legal/business document outlining the course of a patient's medical care,
- to provide statistical and factual information for hospital administration and medical staff committees, for hospital accreditation, licensing and other regulatory bodies, and
- for medical research.
Patient’s have the right to receive a copy of their medical records or inspect them, but requests must be made in writing using the forms accessible from this page. Please see the linked forms to the right. You may print the form, fill it out and return to the Medical Records department at the specific hospital you visited. Medical Records will verify your identity by photo I.D., signature or pertinent questions. We do this to protect the privacy and confidentiality of your records.
