Siena Campus Patient Security Incident
The information that follows applies to patients seen at the Siena Campus between 2001 and 2004.
At St. Rose Dominican Hospitals we understand that our patients’ personal information is important, and we are committed to protecting it. This commitment includes notifying patients if we believe that the security of personal information may have been compromised.
On October 1, 2009, we discovered that information from printed medical records for 309 patients seen at our Siena Campus was mistakenly forwarded to a title company by the facility used to provide secure off-site storage. We have determined that patient names, social security numbers, dates of birth and medical record numbers were among the information forwarded.
We have investigated this incident and retrieved the information. There is no indication that the information was used for any illegal purposes. Nevertheless, in accordance with the law, we are posting this notification. In addition, we are working with the record storage facility to determine how this incident occurred and to ensure that it never happens again.
St. Rose Dominican Hospitals has sent letters to all patients whose personal/sensitive information may have been exposed. If you were seen at our Siena Campus between 2001 and 2004 and have not received a letter, please contact St. Rose Dominican Hospitals at 1-888-568-7675 to find out if your information was involved and learn about steps you can take to protect yourself against identity theft.
We sincerely regret that this incident occurred and apologize for any inconvenience it may have caused our patients. We are taking measures to prevent any similar incidents in the future.
